Incident Report
The following form should be used to report all incidents involving USF students, staff and/or faculty occurring outside the United States:

If you need help while abroad on a USF-related trip:
1. In a life-threatening emergency, first call local police, fire, or medical authorities.

Incident Reporting While Outside of the United States
Any USF traveler who experiences or witnesses a serious medical or safety incident while abroad should report the event. Accurate information is vital to helping the University to support the health, safety and welfare of students, faculty and staff while traveling abroad.
When should I file an Incident Report?
After all urgent issues are addressed and preferably within 72 hours of the event, please report the incident to the Education Abroad Office at USF. Reportable incidents include illnesses and injuries (needing a health care professional), accidents, crimes of all kinds, missing persons, harassment, property damage, and incidents that forced a significant itinerary change.
How do I file an Incident report?
Option A. (Preferred) Access and complete the incident report through the link at the top of this page.
Option B. Send an email to: EAAssist@usf.edu with the subject line "Incident Report" and as much information as possible in the body of the email.
Option C. If you do not have access to email, you may report an incident or request assistance by calling the Education Abroad Duty Officer at #: 813-317-5815 (worldwide). This phone is monitored 24/7 but if your call is not answered immediately you can leave a message and a representative from Education Abroad Office will contact you as soon as possible.
Abbreviated Report Format – Include as much detail as possible.
I. WHO
a. Individuals or groups involved in the incident.
b. What is the individual’s or group’s University of South Florida affiliation?
c. Are the involved parties participating in a specific USF program?
II. WHAT
a. Description of what happened.
b. Was it a medical or security incident, or both?
c. Were there any factors that contributed to the incident (alcohol, weather, etc.)?
III. WHEN
a. Date and time of the incident.
b. What was the activity at the time of the incident (e.g. field trip, free time, research, etc.)?
IV. WHERE
a. Address where the incident occurred (be as specific as possible).
b. Any relevant context about the location (e.g. in front of student’s lodging, on street frequently traveled by students, etc.)
V. ACTION TAKEN
a. What action was taken at the time (e.g. contacted CISI Insurance, US Embassy, sought medical attention, called the police, etc.)?
b. What action still needs to be taken (e.g. counseling for victim, replacing a stolen passport, etc.)?
VI. CONTACT INFORMATION
a. Contact information for individual reporting incident
b . Contact information (if possible) for those involved in the incident
c. May we contact you with questions or to request additional information?
FOR FACUTLY-LED PROGRAMS
As soon as possible, but no later than 72 hours after a serious incident, faculty members should complete a copy of the USF incident report through the link at the top of this page.
HOW WILL THIS INFORMATION BE USED?
Incident reports will be used by the USF’s International Risk and Safety Analyst to monitor the safety and security of University travelers, and to help us prepare and advise future travelers. Collective data and trends may be published, but individual incident reports, including the identity of those involved in or reporting an incident, are treated as confidential and shared only on a need-to-know basis.
