Associate Traveler Application
** This application is intended for non-students who are attending the USF Health program, which include faculty, staff, resident, post docs, GA and accompanying individuals traveling with Students **
USF Education Abroad Scholarship Application Requirements for USF Health students:
 
In order to be considered for the USF Education Abroad scholarship, by the application deadline students must have:
1) Completed the Education Abroad scholarship application(s) in STARS,
2) Initiated travel registration application via the appropriate program link listed above
3) Paid the required $50 non-refundable program deposit by following the below directions
 
All Health students applying for the Education Abroad scholarship can pay the required  $50 (non-refundable) deposit by either check or credit card payment by following the below directions:
(note- failure to follow these instructions exactly as noted, may delay or prevent processing and may jeopardize fulfillment of eligibility requirements for Education Abroad scholarship)

CREDIT CARD PaymentCredit card payments will incur an additional 3% credit card service fee.  Students paying by credit card should call 813-224-7860 which is the HPCC main line and multiple people are able to assist with credit card processing. All major credit cards are accepted. 
 
Students will be required to provide student name and the appropriate program number for their respective program listed above to the HPCC phone agent.
